Currently seeking an RN Coordinator near Baltimore, MD - please see details below:

*Hybrid (mostly remote) - RN must live in Baltimore area as they will need to travel to the office periodically as needed

*Mon-Fri, 8a - 5p

Job Summary: The RN Coordinator is the primary point of contact for patients, coordinating and streamlining all services. They educate patients on healthcare options, answer questions, and build trust. They collaborate with providers and staff to manage schedules, appointments, and other patient needs. The RN Coordinator is responsible for efficient information access and task delegation within the team.

Responsibilities:

• Conduct telephonic nursing assessments and triages according to protocols.
• Manage all aspects of patient care, including appointments, prior authorizations and overall health.
• Facilitate communication between providers and patients, delegating tasks as needed.
• Educate patients on care options and make informed recommendations.
• Explain medical test results, diagnoses, and treatment plans to patient or caregiver.
• Coordinates services, educates patients, and participates in HEDIS initiatives.
• Improves access to care and manages healthcare costs and utilization.
• Participates in weekly care coordination with case management, participates in creating patient care plans.
• Assesses and triages immediate health concerns.

Qualifications:

• Active MD Compact RN license
• 3+ years RN experience
• Previous Telephonic RN Coordinator or RN Case Management experience
• Prior clinical experience in palliative care, hospice, home health, oncology, ICU, geriatrics is preferred.
