Key Responsibilities:
Conduct in-home health assessments to evaluate patients' needs.
Develop personalized care plans based on assessment findings.
Utilize UAS technology to monitor and manage patient care.
Educate patients and their families on self-care, medication management, and wellness.
Collaborate with healthcare teams to ensure continuity of care.
What You'll Need:
RN License: Current New York state nursing license.
Experience: At least 2 years of nursing experience, ideally in home care or community health settings.
UAS Certification: Certification or willingness to obtain UAS certification.
Location: This role is based in The Bronx, New York.
